OKLAHOMA CITY AOTH FOOD & BEVERAGE VENDORS

Thank you for being a food & beverage vendor with Affair of the Heart in OKC. As a Food & Beverage Vendor, you will need to submit your COI (Certificate of Insurance, more info below), Health Department License/Facility # (if applicable), and your electrical needs or notice that electricity is not required (via the form below) upon booking in the show. Please note the deadlines in your Vendor Kit for submitting this information.

Prior to booking you should complete the OKC Fair Park Food/Beverage Booth and /or Sampling Request – Click Here. Once you receive approval from the OKC Facility for your participation, you can reach out to secure your booth.

Food & Beverage Vendors License & Insurance

Please upload your health department license & Certificate of Insurance (COI). Please refer to your contract & the Vendor Kit Manual for insurance requirements. Your COI dates must cover the full move-in through move-out dates for the show you're submitting information on. Please be sure you're submitting the correct COI for the correct show/city.

For the COI, per contract please list the following as additionally insured:The State Fair of Oklahoma, INC., The City of Oklahoma City, and the Oklahoma City Public Authority
